Portal venous gas entrapment secondary to mesenteric ischemia after withdrawal of intra-aortic balloon pump: A case report with a rare ultrasound image

چکیده
After coronary artery bypass graft (CABG), mesenteric ischemia is a rare but dreadful complication with high mortality rates. Prophylactic use of intra-aortic balloon pump (IABP) is a common practice in patients with severe coronary artery disease awaiting CABG. During withdrawal of the IABP, there is a risk of dislodging atheromatous debris, thus causing peripheral and mesenteric emboli, which can cause mesenteric ischemia. Here we are reporting on a case where a patient had an IABP installed after a ST-elevation myocardial infarc. tion (STEMI) while awaiting CABG surgery. After surgery, the IABP was eventually removed, but rising lactates led to further investigation. The abdominal computerized tomography (CT) scan showed extensive bowel pneumatosis and portal venous gas entrapment. The patient died despite aggressive non-surgical management. This case shows a fatal complication of IABP use in a high-risk patient undergoing CABG surgery and highlights the need for careful aorta and peripheral vascular assessment prior to IABP device insertion to avoid intestinal compromise.

Intestinal cholesterol embolism resulting from intra-aortic balloon pumping: a case report
INTRODUCTION Intra-aortic balloon pumping is used in elective percutaneous coronary intervention for increasing coronary blood flow. However, intra-aortic balloon pumping may decrease visceral blood flow and cause mesenteric ischemia by visceral artery obstruction. CASE PRESENTATION We report the case of a 79-year-old Asian man in whom elective percutaneous coronary intervention was performed...
متن کاملManagement of Intra-Aortic Balloon Pump Rupture and Entrapment.
While the intra-aortic balloon pump (IABP) itself has many useful and life-saving indications, it has a high complication rate, up to 30% in some studies. This report discusses the surgical removal and subsequent vascular repair in a patient with a ruptured and subsequently entrapped IABP and reviews the literature on the presentation and management of this rare but major complication.

Four days after admission he ultimately consented to appendectomy and abdominal washout. Follow-up imaging to evaluate for intra-abdominal...
متن کاملThe Evaluation of Survival in Patients Who Need Intra Aortic Balloon Pump (IABP) after Cardiac Surgery
Introduction: Insome cases, assist devices are required to ensure an adequate cardiac output after cardiac surgery. Intra-aortic balloon pump (IABP) is the most commonly used cardiac assist device, which provides first-line support for patients with heart failure. The aim of this study was to determine factors affecting the mortality rate of patients receiving IABP. Materials & Methods: In thi...
